Theremax with Paia midi board

Hello,
I have installed a midi board sold by PAIA into my PAIA Theremax. The Theremax works excellently! The midi board does not. I emailed the tech help at PAiA and was given some specifics, but none that seem to pin down the CV inputs of the midi board that will allow the Theremax to control the midi instrument's pitch and volume.
The PAIA midi board that I have is the Model 9201. It is a convertible model that allows both midi drum control and control voltage to midi applications. It seems to be advertised that way, at least.
With the Theremax connected to the 9201 board and performing with the Theremax, the midi data light blinks, but the midi output to a 'midi-on' signaled keyboard, (Roland XP-10), has no effect.
All circuit board connections are correct, no solder bridges, jack wiring is correct. The control voltage to Midi aspect of this module does not work.
More technically, the 'J5' connection information for control voltage input to the Model 9201 that I received from the PAIA Tech Dept was unspecific, if not down-right vague.I was not given the proper connections to apply this module to be used effectively with the Theremax.
If any have successfully combined the PAIA Theremax and the Midi-Control Voltage Model 9201 into a working midi controller, I would appreciate a word from a Thermax sage.
