Even though I've been a fan for ages, I just now realized that a theremin is heard in the classic Wayne and Schuster Shakespearian Baseball Skatech. I'm almost entirely certain that it is a theremin that begins playing when Wayne is hit on the head by a baseball.
Luckily theres a link available on youtube!

RichieC,
How Hilarious! I guess I was to young to remember seeing these great skit comics.
I think that the head-bump-sound could indeed be a Theremin, or perhaps, an expertly played theatrical slide whistle, or even an Ondes Martinet.

Good Luck!

I have to wonder... from the sheer regularity of the vibrato (and stable intonation on the sustained pitches), I suspect it's an analog synthesizer patched to imitate a theremin. (And none too shabby, I must say.)
